RESALE CERTIFICATE

Please send in a signed and completed resale certificate to sales@rebodyactive.com.

Please follow all of the following guidelines. Once you are finished filling out the form, please upload it. Resale certificates not meeting these guidelines will not be processed.

All resale certificates must be made out to Motion Corporation, DBA Rebody as the vendor/seller
Name of Buyer fields should be completed using the company name on your Rebody account
All fields must be completed, including all descriptions
"For resale" must be included in any general descriptions of items/property being sold
Resale certificates must be currently dated

Please do not send in any resale certificates previously sent to other vendors.
